Zur Klärung der Frage, bei welchen Patienten Myalgien Symptom einer Myositis sind und in welchen Fällen ihnen eine nicht entzündliche Pathogenese zugrunde liegt, erfolgte in der vorliegenden Arbeit die Analyse der Häufigkeit bestimmter klinischer Untersuchungsbefunde sowie deren Korrelation mit entzündlichen Muskelbiopsiebefunden. Anhand der gewonnenen Daten wurde versucht, das Vorliegen einer Myositis anhand eines präbioptischen Modells vorherzusagen. Die Suche nach präbioptischen Prädiktoren eines entzündlichen Muskelbiopsiebefundes ist in Anbetracht der Tatsache bedeutsam, dass eine Muskelbiopsie als invasiver Eingriff das äußerste diagnostische Mittel darstellt und selbst in speziellen Muskelzentren häufig Normalbefunde oder diagnostisch unspezifische Befunde zeigt.
Myalgia is a common symptom in several neuromuscular disorders but also frequently found in rheumatic and endocrine disorders, systemic infections including viral, bacterial and parasitic diseases, non-inflammatory disorders like fibromyalgia-syndrome and psychiatric diseases. It is important to differentiate rather harmless causes of myalgia like muscle soreness after physical workout from vital causes like inflammatory myopathies regularly associated with malignoma. Histopathology is gold standard in diagnosis of most neuromuscular disorders including inflammatory myopathies, but muscle biopsy is an invasive procedure possibly causing complications. Even in specialized centres muscle biopsies often show no diagnosis or an unspecific diagnosis (6-9). In order to predict an inflammatory muscle biopsy outcome in patients with myalgia we analysed 449 patients and studied possible pre-bioptic markers for their suitability of correct prediction.
