A 48-year-old male with no known pre-existing conditions presented to the emergency room with fever (40°C), myalgia, dyspnea and cough for 7 days. The patient was in stable cardiopulmonary conditions with reduced breath-sounds over the left lung and an oxygen saturation of 96% on ambient air. C-reactive protein (CRP), lactate dehydrogenase (LDH), Glutamat-Oxalacetat-transaminase (GOT)/Glutamat-Pyruvat-transaminase (GPT) and -Gamma-Glutamyltransferase (GT) were elevated to 324 mg/l, 352, 64, 53 and 202 U/l, respectively, while leukocytes, thrombocytes, creatinine and creatin kinase were within normal ranges. A severe acute respiratory syndrome coronavirus 2 PCR on a nasopharyngeal swab was negative. A chest X-ray revealed left-sided consolidations. Ampicillin/sulbactam 3 × 3 g i.v. daily was started empirically for community-acquired pneumonia (CAP) and he was admitted to the infectious disease ward. The next morning his oxygen saturation dropped to 88% under 6L nasal oxygen. Renewed assessment revealed that the patient had just returned from a family visit.
